Contracts and Waivers

These electronic form(s) replace the paperwork previously signed in studio at the time of your session.  They need to be completed at the time of booking, and not later than 48 hours before your session.  Failure to complete the required paperwork may result in your session being cancelled or rescheduled, at my convenience.  In the event that you are a repeat customer and have already filled out an electronic contact form with me, you do not need to complete the form again *UNLESS any of your contact information has changed.

If you are booking a cake smash session, please also complete this Cake Smash Waiver (in addition to the contract above).  You will need to complete this form for each child that your schedule a cake smash for.
